Family Case Management is an income-based program that promotes healthy, self-sufficient families through supportive services for pregnant and parenting women and infants. Services include referrals to medical and multiple social service agencies as well as prevention and health education related to pregnancy, childbirth, and child development.
The goals of the Family Case Management Program are to:
- Provide access to primary health care

Our case managers work with doctors, prenatal clinics, hospitals, and other specialty medical providers to ensure adequate health care for their clients.

- Identify and resolve access barriers
|

Our case managers collaborate with community agencies to address client barriers in accessing housing, food, child care, transportation, mental health needs and substance abuse services.

- Provide health education to all eligible clients
|
|
Our case managers work closely with clients to provide accurate health education related to pregnancy as well as infant and child development.

FCM strives to ensure a positive outcome for all pregnant women served by our program. Since 1990, the state's infant mortality rate has decreased by 23%.
