The Family
Case Management Program (FCM) helps families with a pregnant woman, infant or
young child to obtain health care services as well as other assistance they may
need to have a healthy pregnancy and to promote healthy development in children.
The goals of
the Family Case Management Program are to:

Provide
access to primary health care |

Our case managers work with doctors, prenatal clinics, hospitals,
and other specialty medical providers to ensure adequate health care
for their clients.

Identify and resolve access barriers |

Our case managers collaborate with community
agencies to address client barriers in accessing housing, food,
child care, transportation, mental health needs and substance abuse
services.

|
Provide
health education to all eligible clients |
|
|
|
Reduce
infant mortality |
