This
program offers a Comprehensive Care Coordination Assessment to assist with
linking a senior to any community service that is needed to keep the person as
independent as possible and enable them to remain in their own home. The Case
Manager will develop an individualized plan of care based on the clients needs.
Ongoing monitoring and follow-up of each client is provided. The person
must be 60 or over and live in Macon County to be eligible for the assessment.
There are no income or asset guidelines for an assessment.
