This
program provides a consultation and an assessment by a Case Manager for anyone
that is considering nursing home placement or discharge from a nursing home.
The Case Manager will provide the person with all choices related to in home
services that are available in the community. There is no financial eligibility
for this consultation. This assessment is mandated by law for anyone that is
going into a nursing home in Illinois. This program is sponsored by the
Illinois Department on Aging.
