This is a program that is sponsored by the Illinois Department on Aging. It allows someone that is over the age of 60 to remain in their own environment and helps to secure their independence as long as possible. The program offers Homemaker services, Adult Day Care, Home Delivered Meals, Senior Companions, and Emergency Home Response. Homemakers can assist with personal care, laundry, housework, transportation, meal preparation, and supervision. Home Delivered Meals provide one nutritious meal 5 days a week. The Adult Day Care service is located on the campus of St. Mary’s Hospital and provides transportation to and from the center. The Adult Day Care is open Monday – Friday and provides breakfast, lunch, and snacks and activities throughout the day. The center is staffed by a Registered Nurse that may also help with personal care and administering of medications. Senior Companions provide the client with companionship and socialization. The Emergency Home Response Service is an assistive technology device that is worn and must be activated by the client in an emergency such as falling. To be
eligible for the Community Care Program a functional assessment must be
completed by a Case Manager. Asset guidelines due apply. There is a sliding
scale fee based on the person’s monthly income.
