This
program provides information, referrals to community resources, and access to
long term care services. Anyone over the age of 60 and their families and anyone
over the age of 18 with a physical disability and their families are eligible
for this program. There is no financial eligibility for these free
consultations. Information provided could include housing options, in home care
services, assistance with completing applications for Circuit Breaker, Medicaid,
Low Income Subsidy, and assistance with Medicare Part D. This program is
sponsored in part by the Administration on Aging, Centers for Medicare and
Medicaid, the East Central Illinois Area Agency on Aging, and the Illinois
Department on Aging.
