Medication Management & Wellness

This program offers assistance to seniors who have difficulty managing medications. The goal of this program is to promote independent living for seniors who are at risk of losing independence due to inability to manage medications properly. Seniors are provided a registered nurse who will go to the seniors’ home to organize, monitor and order medications, serve as a liaison between the senior and their doctor and pharmacy, and to educate seniors regarding their overall health. The RN can assist diabetics with filling insulin syringes and monitoring blood sugar and diet. There are no income or asset limits for the medication management program; monthly donations are suggested. No senior will be turned away for inability to pay.
