Targeted Intensive Pregnancy Case Management

(TIP)

Intensive Prenatal Case Management is a new initiative to improve the health of newborns. The TIP case manger works closely with pregnant women who are at risk of having a low birth weight or premature infant and ensure that they have access to a physician.

Women are also linked to risk-reduction services such as:

  • Smoking Cessation
  • Substance Abuse Treatment
  • Mental Health Counseling
  • Domestic Violence Services

To ensure that women receive individualized assistance, each staff person works with no more than 40 families at any one time. Case managers must have at least two face-to-face contacts with each client each month the client is enrolled.
